Rehabilitative Care through WSIB
If you have been involved in a Workplace Injury and your employer provides, or is required to provide, Worker’s Compensation (also known as Workplace Safety and Insurance Board), you can apply to have the cost of your rehabilitative treatments covered.
To receive coverage for your health service treatments through the Workplace Safety and Insurance Board (WSIB), it is important that you complete certain steps and documents prior to visiting our clinic.
Steps and Documentation Required Prior to Visiting Us
If you would like all aspects of your rehabilitative treatment to be covered through WSIB, you will need to have their approval which can take some time.
To expedite the process of having your claim approved it is important that you report your injuries to WSIB as soon as possible.
The following are the steps to get claim approval
Notify your employer about your injury.
•Ask your employer to complete a Form 7 (Employers Report of Injury) and sent to the WSIB.
•You will be required to complete a Form 6 (Workers Report of Injury) which has to be sent to the WSIB.
•Visit your Health Care Practitioner and have a Form 8 (Health Professional's Report) completed and sent to the WSIB.
•Contact WSIB to check on the status of your claim number.
•Once you have the above information, you should be able to have the cost of your treatments covered through WSIB.
WSIB utilizes 3 main Programs of Care(POC) for Musculoskeletal Injury
(1)Workers with low back pain will be treated in the LB Program of Care,
(2)Workers with a shoulder injury will be treated in the SH Program of Care and
(3)Workers with other musculoskeletal injuries will be treated in the MSK Program of Care
